Care Management

Care Management Options

  

The AOWN Care Management program is designed to provide identification, coordination, and facilitation of community services to assist elderly to remain in their own homes. We provide a proactive assessment to determine strengths and needs of the client.

The Care Managers conduct a complete assessment of each individual to identify needs, develop and recommend a plan of care, help arrange for appropriate community resources, and provide regular follow-up to assure satisfaction and to identify changing needs. This program does not provide skilled nursing services.

The CMU, by State statute, must determine the individual's ability to cost share in its provision. Each person's monthly income is applied to a sliding fee scale to assess their share. 

The program is available to all older residence of western Nebraska. Referrals are received from any source. If you would like further information on this program, contact the AOWN.
